Video Lending Form

How to Order:

Please submit the following form or download the PDF version of the video lending form and mail, fax or e-mail your request indicating the desired title(s) and correct delivery information for your organization.  We will lend the video(s) to you for a 2-week period, after which you are responsible for returning the video(s), either by Canada Post with proof of delivery, or in person at the Centennial Building main desk (670 King St.).
